Schedule Appointment Online
Booking your appointment online is the fastest way to make an appointment, and allows you to ensure you are scheduled for the next available appointment by location, service, and/or provider. Click the button below to schedule your appointment.
You may also submit a request for an appointment through our Request Appointment form, or by calling our office during regular business hours at (904) 404-8555. We are open Monday through Friday from 8:00am – 4:00pm. We do not offer weekend appointments. If for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible.
Insurance and Billing
We accept most traditional insurance plans. Please contact our office to verify acceptance of your plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure. We are happy to file insurance for your reimbursement as long as you are free to choose your own dermatologist.
Payment Options
We accept checks, cash or credit cards. We also offer a flexible payment plan. Please see our Financial Coordinator for details.
